Campaign 09V-388: the floor mat
Filed with NHTSA on October 5, 2009, this is the larger of the two acceleration campaigns and, by unit count, one of the largest single recalls in American automotive history. The federal record puts the affected population at 4,445,056 vehicles.
The defect was not the pedal itself but the space around it. On the covered models, a driver’s-side floor mat that was unsecured, stacked on top of another mat, or simply the wrong part for the car could ride forward and hold the accelerator down. Because the pedal was mechanically trapped rather than electrically commanded, lifting off had no effect — which is what made the failure so frightening and so hard for drivers to describe afterward.
The remedy ran deeper than a floor mat swap. Toyota reshaped accelerator pedals, modified floor surfaces, and on many models installed a brake-override system that cuts throttle when the brake is applied at the same time.
Campaign 10V-017: the sticking pedal
Three months later, on January 21, 2010, Toyota filed a second and entirely separate defect report covering 2,230,661 vehicles. This one was about the pedal assembly.
A friction device inside the pedal, fitted to control the feel of the throttle, could wear and absorb moisture in a way that made the pedal harder to depress, slower to spring back, or — in the worst case — able to hold a partially depressed position on its own. The affected assemblies came from a single supplier, which is why the recall covered a specific slice of the model range rather than every car of that year.

What the NASA investigation actually found
This is the part of the story that almost never survives into the retellings, and it is the single most useful thing to know if you are looking at a Toyota of this era.
A popular theory held that the real culprit was the electronic throttle — software or electromagnetic interference commanding acceleration nobody asked for. Congress asked NHTSA to settle it, and NHTSA brought in NASA engineers with backgrounds in computer-controlled electronic systems, electromagnetic interference and software integrity. The study ran ten months.
NASA analyzed more than 280,000 lines of Toyota software code. Hardware and systems engineers stripped and tested throttle components at the Goddard Space Flight Center in Maryland. At a facility in Michigan, engineers bombarded complete vehicles with electromagnetic radiation to see whether radiation could induce a malfunction. NHTSA ran further mechanical testing at its Vehicle Research and Test Center in East Liberty, Ohio.
For a buyer, the practical consequence is simple. The defects that caused this were physical, they were identified, and they had physical remedies that a dealer either performed on your VIN or did not. That is a question the federal lookup answers in seconds — unlike a suspected software gremlin, which nothing could ever have closed.
What changed across the industry afterward
The episode reshaped federal vehicle safety policy well beyond Toyota. On the strength of the findings, NHTSA moved to propose rules requiring brake-override systems, standardizing how keyless ignition systems behave, and requiring event data recorders in passenger vehicles. It also opened broader research into the reliability and security of electronic control systems generally.
Toyota paid $48.8 million in civil penalties over federal investigations into whether it had reported the defects to the government in a timely way. That figure is worth separating from the defects themselves: the penalties were about disclosure timing, not about a further mechanical fault.
The brake-override system that came out of this is now unremarkable equipment. If you are driving a Toyota built since the middle of the 2010s, the car will cut throttle when you press the brake and accelerator together — a direct inheritance from these campaigns.
Why the repair costs you nothing
Federal law puts the cost of fixing a safety defect on the manufacturer, not the owner. Parts and labor are covered, and the obligation follows the vehicle rather than the original buyer — a fifth owner has exactly the same claim as the first.
There is one boundary worth knowing, and it is commonly misread. The 15-year limit is measured to the recall rather than to the repair: the free-remedy requirement applies where the vehicle was first sold less than 15 years before the defect was determined. The acceleration campaigns were filed in 2009 and 2010, comfortably inside that window for the cars they cover, so the remedy on them remains an obligation rather than a favor. What would fall outside is a defect determined today on a car sold long enough ago to clear the 15 years.
Lexus, Scion and how the campaigns split
Lexus is Toyota’s luxury marque and Scion was its youth brand, and both were swept into the acceleration campaigns alongside Toyota-badged cars. The campaigns are filed under their own marques rather than folded into one Toyota entry, so a search by brand name can easily miss half the picture.
This is the strongest argument for searching the number rather than the nameplate. The VIN is indifferent to marketing divisions; it returns whatever is attached to that vehicle regardless of which badge is on the trunk.
Reading an open recall on a used Toyota
An open recall on a used car is common and, on its own, not a red flag about the seller. Notification letters chase registered owners, owners move, cars change hands privately, and the letter ends up at an address three owners out of date. The campaign does not expire from neglect — it simply sits on the VIN, waiting.
What matters is what you do with the information. An open campaign is a free repair you inherit along with the car, not a defect the seller is hiding from you. It is also a reasonable thing to raise before money changes hands, because the seller can book it as easily as you can.

If a campaign comes back open
Call any Toyota dealer — it does not have to be the selling dealer or the nearest one — give them the VIN, and ask for a recall appointment. You are not buying anything, so there is nothing to negotiate.
If the notice carries specific guidance about driving the vehicle in the meantime, follow it literally. Most campaigns carry none. The ones that do are the ones where the agency assessed the risk as immediate rather than statistical, and the wording is chosen deliberately.
If you already paid for a repair that a recall later covered, ask about reimbursement. Manufacturers operate reimbursement plans for exactly this situation, though they usually run to a cutoff date and require the original receipt.

The first property is intensity, and it is low. Those 40 campaigns across seven records work out at 5.7 per record, against a figure of 12.9 for the Ford records and 11.0 for the GMC ones. It is not quite the lowest we hold: of the twelve manufacturers we carry three or more records for, only Kia sits below Toyota, at 5.6. What the 5.7 does say is that a marque with this many campaigns in the public memory does not produce a dense record on the model years we hold.
The second property is variety, and it is the highest we hold. Those same 40 campaigns are spread across 19 distinct component categories— more than any other marque, including Ford, which produces 116 campaigns across 18. Ten of the nineteen carry a single campaign each.
Put together: many different things recalled, few of each. The largest single category is equipment on 6, then a three-way tie on 4 between air bags, gasoline fuel system and hydraulic service brakes. Nothing dominates.
What a recall check will never show you
A recall lookup answers one narrow question extremely well: does the manufacturer owe this vehicle a free safety repair right now? It is silent on everything else.
It will not tell you whether the car has been crashed, whether the title carries a brand, whether the odometer reading is honest, or whether a lien is still recorded against it. Those live in different systems entirely, and a clean recall result says nothing about any of them.
